Requesting Your Disability File

  1. Contact SSA: Call the SSA at 1-800-772-1213 or visit your local SSA office to request a copy of your disability file. 
  2. What You’ll Need: Provide your social security number, name, birth date, and mailing address. If visiting in person, bring identification like a driver’s license. 
  3. Receiving Your File: The file will be mailed to you on a disc. If you can’t use a CD-ROM, a printed file may be available for a fee. 
  4. Expected Wait Time: Typically, it takes about 1-2 weeks to receive your file, but plan for up to four weeks. 
  5. Disc Password: The disc is secured with a password, which is the first four letters of your first name (lowercase), a “#”, followed by the last four digits of your Social Security Number. For example, for John Smith with SSN 123-45-6789, it would be john#6789. 
  6. File Contents: Your file will include your disability application, appeals, work history, and all medical records requested by DDS or provided by you​​.
